In consideration of receiving a tattoo or piercing from Neon Tattoos & Piercings (together with its employees, apprentices and agents). I agree to the following:
Understanding of Risks
I have been fully informed of the inherent risks associated with receiving a tattoo or piercing, including but not limited to:
- Infection, scarring, prolonged healing, and complications from improper aftercare.
- Allergic reactions to tattoo ink, piercing jewelry, antiseptics, latex gloves, or other materials used during the procedure.
- Possibility of keloids, excessive bleeding, or skin sensitivities.
- Permanent alteration to my body, and in the case of tattoos, the potential fading or blurring over time.
- The risk of rejection, migration, or embedding of piercing jewelry.
Assumption of Responsibility
I fully accept and assume all risks arising from my tattoo or piercing application and release the studio, artists, and piercers from any liability, including personal injury, direct or consequential damages.
Medical Disclosure
I confirm that:
- I am not under the influence of drugs, alcohol, or any mind-altering substances.
- I have disclosed any known medical conditions that may affect healing (e.g., diabetes, epilepsy, hemophilia, heart conditions, blood-thinning medications, skin conditions, or compromised immune system).
- I am not pregnant or nursing.
Aftercare & Healing Responsibility
- I have been provided with verbal and/or written aftercare instructions and understand my responsibility in following them.
- I acknowledge that failure to follow aftercare guidelines can result in infection or other complications.
- If touch-up work is required due to my own negligence, I will be responsible for any additional costs.
Legal Understanding
- I acknowledge that a tattoo is permanent and can only be removed by laser or surgical means, which may not fully restore my skin.
- I confirm that any spelling, symbols, or artwork are correct, and I take full responsibility for the design chosen.
- I understand that variations in color and design may occur, and tattoos may fade over time.
Consent to Procedure
- I voluntarily consent to the tattoo or piercing procedure without coercion or pressure.
- I understand that I may stop the procedure at any time but will not be entitled to a refund.
